Complete the form to have the Co-op option added back onto your academic profile after having declared into one of the following diploma programs:
- Computer Information Systems
- Public Relations
- Marketing Management
You will need to include a screenshot from KPU Online Self Service (OSS) where the status of your request to declare program is completed.
